Patient Rights & Responsibilities

RIGHTS

1. To be given good quality healthcare in a safe setting without discrimination regardless of age, sex, gender, religion, ethnicity, political affiliation, disability or capacity to pay.

Mabigyan ng de-kalidad na serbisyong pangkalusugan sa isang ligtas at maayos na pasilidad ng walang diskriminasyon anuman ang iyong edad, kasarian, relihiyon, etnisidad, kasapiang political, kapansanan o kakayahang makapagbayad.


2. To be assigned a competent physician, and know about financial assistance.

Mabigyan ng mahusay na doctor at malaman ang bayarin sa ospital at doctor at makatanggap ng impormasyon hinggil sa posibilidad na tulong pinansiyal.


3.To be allowed to notify chosen relatives and doctor about hospital admission.

Maipaalam sa mga kapamilya o mga kamag-anak at sa napiling doctor ang tungkol sa iyong kalagayan at pagkakapasok sa ospital.


4. To be accompanied during hospital stay provided it will not pose risk to the rights, safety or health of others.

Magkaroon ng kasama sa iyong pananatili sa ospital maliban lamang kung nakokompromiso nito ang karapatan, kaligtasan o kalusugan ng iba.


5. To be allowed to exercise spiritual and cultural beliefs.

Malaya kang isagawa ang iyong espiritwal at kultural na paniniwala na umaayon sa mga kakayahan at mga tuntunin ng ospital.


6. To be properly informed before giving express consent in any procedure.

Malaman at makapagbigay ng pahintulot o tanggihan ang anumang pamamaraan o pananaliksik na di-emergency.


7. To be secured of the privacy and confidentiality of medical records.

Makasiguro sa pagiging pribado at kumpidensyal ng iyong ulat medical alinsunod sa batas.


8. To be accompanied during physical examination.

Maaari kang humiling ng kasama habang isinasagawa ang pisikal na eksaminasyon.


9. To be represented in behalf of them.

Makapagtalaga ng taong magrerepresenta sa iyo, kung kakailanganin.


10. To be informed of the complaints and grievance process.

Malaman ang proseso ng pagrereklamo at ipahayag ang mga hinaing ng walang takot sa igaganting paratang o paghihiganti. Ikaw ay hinihikayat na direktang makipag-usap sa iyong health care provider na nangangalaga sa iyo.

RESPONSIBILITIES

1.Shall provide adequate, accurate, truthful and complete information about all matters pertaining to his/her health. This includes past illnesses, medications, advance directives and other health related matters. He/she shall report any changes in his/her condition to his/her physician or healthcare team.

Magbigay ng wasto at kumpletong impormasyon tungkol sa iyo at sa iyong kalusugan kabilang na ang mga kasalukuyang reklamo, nakaraang problema sa kalusugan at mga pagbisita sa ospital, mga gamut na iniinom at iba pang impormasyon sa iyong palagay ay kailangang malaman ng iyong mga tagapangalaga.


2. Shall ensure that he/she understands the purpose and cost of any proposed treatment or procedure before deciding to accept it.

Tiyakin na nauunawaan ang lahat ng pamamaraan, pagsusuri, halaga at paggamot na ipinaliwanag ng mga kinauukulan bago magpasyang tanggapin ito.


3.Shall follow the treatment plan to which he/she agreed upon.

Sundin ang napagkasunduang plano sa pangangalaga.


4.Give consideration for the rights of other patient’s and healthcare providers by following hospital rules and regulations concerning ethical and respect to other’s privacy.

Magbigay ng pagpapahalaga sa mga karapatan ng kapwa pasyente at kanilang tagapangalaga sa pamamagitan ng pagsunod sa lahat ng mga alituntunin ukol sa karapatan ng bawat pasyente.


5.Inform the attending physician should you desire to seek second opinion or to consult another physician.

Magpaalam sa kasulukuyang sumusuring doctor kung nais humingi ng pangalawang opinyon mula sa ibang manggagamot bago lubusang magpasya.


6. Shall ensure the financial obligations to my healthcare are fulfilled or settled.

Maninigurado na tutuparin ang pinansyal na obligasyon sa ospital ayon sa kontratang nilagdaan.


7.Ensure the integrity and authenticity of medical record and understand that any false information is a criminal act under the penal code.

8. Leave the valuables and personal belongings at home or have your family members take them home.
